Pharmacist perspectives on emerging T cell–engaging bispecific therapies in cancer therapeutics.

Abstract

The article focuses on the pharmacology, efficacy, safety, dosing, administration, and operational considerations of newly approved bispecific T cell–engaging therapies for cancer treatment, primarily hematological malignancies. Bispecific antibodies are engineered to simultaneously bind a tumor-specific antigen and CD3 on T cells, activating an immune response; five such therapies—epcoritamab, glofitamab, mosunetuzumab, tebentafusp, and teclistamab—have received FDA approval for indications including diffuse large B cell lymphoma, follicular lymphoma, uveal melanoma, and multiple myeloma. These therapies commonly cause cytokine release syndrome (CRS), necessitating specific premedication, monitoring, and hospitalization protocols that vary by agent, highlighting the critical role of pharmacists in managing dosing schedules, adverse event mitigation, and transitions of care. The article emphasizes the importance of documentation, multidisciplinary collaboration, and pharmacist involvement in operationalizing these therapies safely, especially as the bispecific antibody pipeline expands and these treatments become more widely used.
