Abstract
This article focuses on the implementation of gender-affirming mental health care for transgender and gender-expansive (TGE) individuals in Southeast Asia, emphasizing the importance of affirming intake processes and integrated mental health support within primary care settings. It highlights the challenges posed by persistent stigma, structural discrimination, and conservative religious and legal frameworks that often marginalize TGE persons in the region. The article discusses how culturally sensitive adaptations that align with regional values of communal harmony and respect for tradition, alongside engagement with progressive faith-based leaders, are crucial for fostering acceptance and trust. It also notes varying policy environments, with countries like the Philippines and Thailand showing potential for supportive reforms, while others maintain restrictive approaches. Successful gender-affirming care in Southeast Asia requires nuanced cultural negotiation, sustained advocacy, and ethical sensitivity to local social and moral contexts.
