Testing for lactase non-persistence in a Dutch population: Genotyping versus the hydrogen breath test.

Abstract

This article focuses on evaluating diagnostic methods for primary lactose intolerance (LI) in a Dutch population with a high prevalence of lactase-persistent alleles. The study compared the hydrogen breath test (HBT), which measures hydrogen production after lactose ingestion, with DNA genotyping of the −13910 C/T polymorphism associated with lactase persistence. Results from 1101 patients showed that DNA genotyping had high accuracy and was more patient-friendly, while HBT was time-consuming, potentially uncomfortable, and less specific, especially since symptom questionnaires added little diagnostic value. The authors recommend replacing HBT with genotyping of lactase-persistent alleles as the primary diagnostic tool for suspected LI in similar populations.
