Abstract
This article focuses on the current service landscape and care needs for persons with dementia and significant violence risk in Victoria, Australia. Using a modified Policy Delphi method, 12 medical practitioners specializing in forensic psychiatry, psychiatry of old age, geriatric medicine, and general practice were interviewed to explore challenges and opportunities in care provision. The study highlights the heterogeneity of this population, the lack of specific services—especially for those in custodial settings—and the complex ethical, legal, and medical issues involved. Five key recommendations were identified, including developing appropriate care settings and pathways, formalizing service agreements between forensic and non-forensic sectors, improving physical environments, enhancing support and education for carers, and considering the establishment of a specialized service. The findings underscore the need for further research and policy development to address the anticipated growth of this population and their unique care requirements.
