Abstract
The article describes the contribution of pharmacists integrated into primary care offices within a community practice health system to two Merit-based Incentive Payment System (MIPS) care improvement activities and to medication adherence gains, which improved the health system's overall Medicare Part C and Part D star rating. It discusses the roles of the pharmacists integrated within the primary care locations of St. Joseph's/Chandler Health System. It explains the quality metrics evaluated.
