Abstract

This article focuses on improving patient safety in dentistry by providing evidence-based guidance for the prevention, recognition, management, and reporting of sodium hypochlorite (NaOCl) injuries during endodontic treatment. It addresses all types of NaOCl-related adverse events and near misses, emphasizing the importance of standard operating procedures (SOPs) to reduce harm, including practical prevention measures such as protective equipment and careful irrigation techniques. The article highlights the need for consistent incident reporting systems to identify patterns and improve safety culture, while recommending transparent communication with patients, including apologies under the duty of candour. Due to limited high-level evidence, the article proposes pragmatic SOPs as interim guidance to enhance patient care and safety in dental practice.
