Abstract
This article evaluates the clinical and financial impact of integrating remote-based clinical oncology pharmacists into four community oncology practices within The US Oncology Network. Over 12 months, oncology-trained pharmacists working at 0.5 full-time equivalents per practice reviewed 5,716 chemotherapy orders, achieving a 57% intervention rate that improved medication safety, regimen optimization, and practice margins by approximately $1.46 million while reducing medication costs by nearly $6 million. The program demonstrated an average return on investment of 415%, highlighting the cost-effectiveness and clinical value of remote oncology pharmacists in resource-constrained community settings. Challenges included workflow integration and communication across different electronic medical records, but collaboration between remote and on-site pharmacists enhanced overall pharmacy services.
