Sidelined by the side-eye: Exploring the effects of nonverbal communication in healthcare services for 2SLGBTQI+ patients.

Abstract

This article examines the role of nonverbal communication in shaping healthcare experiences for 2SLGBTQI+ (Two Spirit, Lesbian, Gay, Bisexual, Trans, Queer/Questioning and/or Intersex+) patients, drawing on qualitative data from the Queer Queering and Questioning (QQQ) project in Ontario, Canada. Analysis of interviews and focus groups with 2SLGBTQI+ patients, healthcare providers, and allies reveals that unconscious nonverbal cues—such as body language, eye contact, and environmental symbols—significantly impact patient comfort, perceptions of acceptance, and willingness to seek care. The study highlights the importance of critical self-reflection and reflexivity among healthcare professionals to recognize and manage unconscious biases, alongside the need for inclusive physical environments and enhanced training on nonverbal communication skills to improve affirming care for 2SLGBTQI+ communities. It also notes that 2SLGBTQI+ healthcare providers experience a dual consciousness as both providers and patients, which informs their understanding of quality care but may carry emotional costs.
