The effect of group-based cognitive behavioural therapy for insomnia in patients with rheumatoid arthritis: a randomized controlled trial.

Abstract

This article focuses on a randomized controlled trial comparing nurse-led group-based cognitive behavioural therapy for insomnia (CBT-I) with usual care in patients with rheumatoid arthritis (RA). The primary outcome, sleep efficiency measured objectively by polysomnography (PSG) at week 7, showed no statistically significant difference between CBT-I and usual care. However, at week 26, CBT-I demonstrated statistically significant improvements in patient-reported outcomes related to insomnia severity, sleep quality, fatigue, depressive symptoms, pain, and the RA Impact of Disease (RAID) score. The trial suggests that while CBT-I may not improve objective sleep measures in RA patients, it has clinically meaningful benefits on subjective sleep and RA-related symptoms, supporting its role as a non-pharmacological treatment option for insomnia in this population.
