Therapeutic failure in leprosy, French Guiana, 2011–2024: Impact of bacillary index.

Abstract
The article focuses on the high rates of treatment failure and relapse in leprosy patients in French Guiana, with an overall failure rate of 43%. The study analyzed 125 patients diagnosed between 2011 and 2014, revealing discrepancies between WHO classifications and histological findings, as well as a significant association between a high bacillary index (BI) and treatment failure or relapse. The findings suggest that the current treatment durations recommended by the WHO may be insufficient, particularly for multibacillary patients with a BI of 3 or more, and propose extending treatment durations to improve outcomes.
