Abstract
This article focuses on addressing health inequities rooted in social determinants of health (SDOH) through a proposed three-part interdisciplinary framework aligned with the Social Work Grand Challenge of "closing the health gap." The framework emphasizes (1) strategic collaboration between macro social work and allied disciplines such as community psychology, (2) embedding community-centered competencies grounded in social justice values, and (3) building power via shared decision making and structural change with marginalized communities. A case study of a trans health equity project illustrates how community-led participatory action research and power sharing can foster transformative social change. The authors argue that dismantling systemic inequities requires redistributing power and co-developing solutions with communities to achieve sustainable health equity.
