Abstract
This article focuses on the recognition and management challenges of severe hemolytic disease of the newborn (HDN) in community hospitals lacking higher-level neonatal intensive care units (NICUs). HDN results from maternal immunoglobulin G antibodies destroying fetal red blood cells, leading to rapid hyperbilirubinemia and anemia in neonates. Management includes phototherapy, intravenous immunoglobulin (IVIG), intravenous fluids, and transfusions, with transfer to tertiary care centers recommended for severe cases. The article emphasizes early detection through cord bilirubin and direct antiglobulin testing, discusses limitations of diagnostic tools, and highlights the need for standardized guidelines and enhanced training in community settings to improve outcomes.
