Role of calprotectin in diagnosis of necrotizing enterocolitis and Doppler US in detecting its severity and outcome in neonates.

Abstract
This article focuses on evaluating fecal calprotectin (FCP) and Doppler ultrasound (US) as diagnostic and prognostic tools for necrotizing enterocolitis (NEC), a severe gastrointestinal emergency in preterm neonates. In a case-control study of 54 NEC cases and 42 matched controls, FCP levels were significantly elevated in NEC patients, with an optimal cutoff of 176 µg/g yielding 87% sensitivity and 97.6% specificity. Doppler US detected decreased bowel perfusion in 77.8% of severe (Stage III) NEC cases, strongly correlating with increased mortality (odds ratio 4.2). The combined use of FCP and Doppler US offers a non-invasive, radiation-free approach for early NEC diagnosis and risk stratification, potentially improving clinical decision-making in high-risk neonates.
