Roflumilast and the Changing Landscape of Seborrheic Dermatitis Treatment.

Abstract
This article reviews clinical trial data on roflumilast, a phosphodiesterase-4 (PDE-4) inhibitor recently approved by the U.S. Food and Drug Administration (FDA) for the treatment of seborrheic dermatitis (SD), a chronic inflammatory skin condition characterized by erythematous, scaly plaques. Two randomized controlled trials demonstrated that once-daily topical roflumilast foam (0.3%) significantly improved Investigator Global Assessment (IGA) scores, reducing erythema, scaling, and itch severity compared to vehicle, with a favorable safety and tolerability profile. Roflumilast's mechanism targets inflammatory pathways at a molecular level, distinguishing it from existing treatments such as corticosteroids and antifungals, which have notable side effects and application challenges. The article concludes that roflumilast is an effective and well-tolerated option for SD, while recommending future large-scale comparative studies to further define its clinical role.
