Abstract

This article focuses on resilience and transition readiness for adolescents living with rheumatic diseases, emphasizing the need for comprehensive, adolescent-oriented care that supports successful transition from pediatric to adult services. It reviews existing literature on resilience programs and transition services, noting significant challenges such as lack of adolescent-specific care, limited resources, and insufficient multidisciplinary coordination globally, with a particular scarcity of data from Sub-Saharan Africa (SSA). The authors highlight that most transition models and assessment tools originate from high-income countries, underscoring the urgent need for context-specific approaches tailored to the unique challenges faced by adolescents with rheumatic diseases in SSA.
