Risk of Urinary Tract Infections in Male Veterans With Diabetes Prescribed Sodium-Glucose Cotransporter-2 Inhibitors Versus Sulfonylureas Across the Veterans Health Administration.

Abstract
This article focuses on comparing the risk of urinary tract infection (UTI) in male Veterans with type 2 diabetes mellitus (T2DM) on metformin who were newly prescribed sodium-glucose cotransporter-2 inhibitors (SGLT2is) versus sulfonylureas (SUs) within the Veterans Health Administration (VHA). The retrospective cohort study found a statistically significant decrease in the odds of outpatient UTI diagnosis and no difference in inpatient UTI diagnosis among those taking SGLT2is compared to SUs. These findings contribute to existing literature suggesting that SGLT2i therapy does not increase UTI risk, despite previous concerns based on their mechanism of action and adverse event reports. Limitations include the study’s focus on a predominantly older, male Veteran population and lack of data on glycemic control and medication adherence.
