Circinate balanitis in HLA B27 spondyloarthrithis: A report of two cases.

Abstract

This article focuses on two cases of circinate balanitis, a mucocutaneous lesion typically associated with reactive arthritis (ReA), occurring in patients with HLA B27-positive spondyloarthritis without the classical ReA triad of urethritis, conjunctivitis, and arthritis. The first case involved HLA B27-positive psoriatic spondyloarthritis confirmed by skin biopsy and MRI findings, while the second case involved HLA B27-positive primary ankylosing spondylitis with similar penile lesions. These cases highlight the diagnostic challenges in distinguishing circinate balanitis within different forms of spondyloarthritis and suggest the need for careful clinical, imaging, and histological evaluation to refine diagnostic criteria and treatment approaches.
