Altered sensation following extrusion of an endodontic file treated by intentional replantation: case report and treatment recommendations.

Abstract

This article focuses on the management of altered sensation caused by the extrusion of an endodontic file into the inferior alveolar nerve canal (IANC) during root canal treatment, presenting a case of a 12-year-old girl treated successfully with intentional replantation. Intentional replantation involved atraumatic extraction, removal of the separated file from the socket, root-end resection, retrograde filling with mineral trioxide aggregate, and replantation of the tooth, followed by antibiotic and steroid therapy. The procedure led to rapid nerve recovery and favorable long-term outcomes, including periapical bone healing and normal tooth function at 15 months. The article emphasizes the importance of prompt multidisciplinary intervention, careful surgical technique, and regular follow-up to manage nerve injuries related to endodontic procedures effectively. A detailed treatment protocol incorporating radiographic evaluation and timely surgical management is also proposed to mitigate neurologic complications.
