Effectiveness of dental arch expansion in the orthodontic treatment with clear aligners: a scoping review.

Abstract

This scoping review examines the effectiveness and predictability of dental arch expansion using Clear Aligner Therapy (CAT) in both children and adults. Based on 33 studies—including systematic reviews, cohort, case-control, and case series—CAT was found to effectively expand dental arches, with greater predictability in the lower arch compared to the upper arch and more reliable expansion in the premolar region than in canine or molar areas. The review highlights that expansion achieved with CAT is primarily dental, with no current evidence supporting skeletal changes, and that predictability varies widely (0% to 99%) due to differing protocols and measurement methods. The authors emphasize the need for randomized controlled trials to provide more definitive clinical guidance on CAT for arch expansion.
