Lenacapavir to prevent HIV infection: current prices versus estimated costs of production.

Abstract
The article focuses on the potential for mass production and affordable pricing of lenacapavir, a long-acting injectable HIV pre-exposure prophylaxis (PrEP) drug. Despite current prices of up to $44,819 per person per year (pppy), analysis of active pharmaceutical ingredient (API) costs and manufacturing processes suggests that generic production could reduce prices to approximately $94 pppy at a scale of 1 million treatment-years and $41 pppy at 10 million treatment-years, contingent on voluntary licensing and competition among generic manufacturers. Clinical trials, including the recently stopped early PURPOSE 1 study, demonstrate lenacapavir's high efficacy in preventing HIV, particularly among young women in sub-Saharan Africa. The article emphasizes the urgent need for voluntary licensing agreements, especially with the Medicines Patent Pool, to enable generic production and equitable access in low- and middle-income countries, where HIV incidence remains highest.
