Creating Dynamic Community Resource Maps for Transition Planning.

Abstract

This article focuses on community resource mapping (CRM) as a collaborative, asset-based process to identify and organize community resources that support the post-secondary transition to adulthood for youth with extensive support needs (ESN). It outlines a four-phase approach—pre-mapping, mapping, taking action, and maintaining/evaluating—to develop dynamic digital CRMs that enhance interagency collaboration among families, educators, and community providers. Dynamic CRMs, often created using platforms like Google My Maps, offer customizable, accessible, and real-time updated maps that can be individualized to meet diverse transition needs, while static CRMs serve as simpler, printable alternatives. The article emphasizes the importance of including youth and families in the CRM process to ensure cultural relevance and usability, and it discusses limitations such as technology access disparities and the need for ongoing maintenance to sustain effectiveness.
