Abstract
This article focuses on a research program based in Wellington, New Zealand, that aims to reframe supported self-management for people with long-term conditions or permanent impairments, particularly among Maori, Pacific Peoples, and individuals with learning disabilities. The study emphasizes a holistic, culturally grounded approach to support that recognizes the strengths and lived experiences of these populations and their families (whanau). Using participatory case studies and mixed methods over a five-year period, the research seeks to develop, implement, and evaluate innovative models of care tailored to community aspirations and contexts. Early phases have involved extensive community consultation and capacity building, with the goal of informing New Zealand health policy and practice to reduce health inequities and promote well-being.
