Iatrogenic Botulism Following Botulinum Toxin Injection in a Child With Cerebral Palsy: A Case Report.

Abstract

This article focuses on a case of iatrogenic botulism, a rare but serious systemic complication following botulinum toxin A (BoNT-A) treatment for spasticity in pediatric cerebral palsy patients. An 8-year-old boy with spastic quadriplegic cerebral palsy developed generalized weakness, bilateral ptosis, dysphagia, and respiratory distress after receiving a high per-kilogram dose of abobotulinumtoxinA (Dysport), despite prior uneventful treatments with onabotulinumtoxinA (Botox). The diagnosis was supported by clinical presentation and improvement following administration of botulinum antitoxin, even though treatment was initiated late. The report underscores the need for careful dosing, patient monitoring, and family education about early warning signs to promptly identify and manage this potentially life-threatening condition.
