Abstract

This study examines insurance coverage and costs of vaginal estrogen formulations across Mid-Atlantic states in 2024, analyzing 342 insurance plans from the top insurers in populous districts. Coverage scores, based on prescription tiers, showed estradiol tablets had the best coverage, while some formulations like Estrace tablets and Vagifem had lower scores. Median costs varied significantly by tier, with tier 1 medications often free and non-formulary drugs costing substantially more. Geographic location and insurer influenced costs, with Delaware residents and those insured by Aetna or Kaiser facing higher expenses. The findings highlight potential financial barriers for postmenopausal women requiring vaginal estrogen therapy in this region.
