Valproic Acid–Associated von Willebrand Disease and Delayed-Onset Orbital Hematoma.

Abstract

This article focuses on a rare case of vision impairment caused by a subgaleal hematoma extending into the orbital subperiosteal space in a 14-year-old boy with cerebral palsy and a seizure disorder treated with multiple antiepileptic drugs, including valproic acid. The patient developed type I von Willebrand disease, a bleeding disorder likely induced by valproic acid, which contributed to the hematoma formation and subsequent orbital complications. Despite surgical evacuation and adjustment of anticonvulsant therapy, the patient experienced persistent optic nerve damage. The report highlights the potential coagulopathic risks associated with long-term valproic acid use and the unusual presentation of orbital hematoma secondary to subgaleal hemorrhage in this context.
