The Role of Capsule Endoscopy in the Management of Pediatric Hereditary Polyposis Syndromes.

Abstract

This article focuses on the role of video capsule endoscopy (VCE) in managing pediatric hereditary polyposis syndromes (HPS), which involve small intestinal (SI) polyps requiring surveillance to prevent complications. VCE is established as a valuable, relatively safe tool for detecting SI polyps in children with Peutz-Jeghers syndrome (PJS) and constitutional mismatch repair deficiency (CMMRD), enabling early intervention to reduce morbidity. Its utility in familial adenomatous polyposis (FAP), juvenile polyposis syndrome (JPS), and PTEN hamartoma tumor syndrome (PTEN-HS) is less clear, with routine VCE surveillance not generally recommended for asymptomatic children in these groups. Technical challenges in pediatric VCE include capsule ingestion and accurate polyp sizing and localization, with future advances anticipated through artificial intelligence and improved technology.
