Cystectomy and lymphadenectomy – we now node how far to go!

Abstract
The article discusses the results of two phase III randomized controlled trials, SWOG S1011 and German LEA AUO AB 25/02, which found that standard lymph node dissection (LND) during cystectomy for bladder cancer is sufficient, with no added benefit from extended LND. The trials showed no improvement in recurrence-free survival or overall survival with extended LND, and in fact, the SWOG trial reported a higher burden of adverse effects in the extended LND group. The studies caution against extending LND beyond the internal and external iliac lymph node chains, suggesting that a more limited LND is preferable. [Extracted from the article]
