Abstract

This article presents updated evidence-based guidelines from the British Association for Sexual Health and HIV (BASHH) Clinical Effectiveness Group for the diagnosis, treatment, and health promotion of gonorrhoea, caused by the bacterium *Neisseria gonorrhoeae*. Key updates include recommendations for routine pharyngeal testing in all individuals with urogenital gonorrhoea and their contacts, revised antimicrobial regimens reflecting rising resistance (notably increased cefixime dosing and restricted ciprofloxacin use), and guidance on managing complicated infections such as disseminated gonococcal infection and ocular gonorrhoea. The guideline emphasizes the importance of culture and antimicrobial susceptibility testing to monitor resistance, outlines diagnostic approaches including nucleic acid amplification tests (NAATs), and provides specific considerations for populations such as those following gender-affirming surgery and pregnant individuals. It also addresses partner notification, test-of-cure protocols, and management of treatment failures, underscoring the need for clinical judgment tailored to individual circumstances.
