The Combined Use of Fine-Needle Aspiration (FNA) and BRAF V600E Gene Testing: Can it Increase the Definitive Diagnosis Rate of Nodules Categorized as Bethesda III for Papillary Thyroid Carcinoma?

Abstract

This study focuses on evaluating the malignant probability of thyroid nodules with indeterminate cytology classified as atypia of undetermined significance or follicular lesion of undetermined significance (AUS/FLUS) and assesses the diagnostic value of combining fine-needle aspiration (FNA) with BRAF V600E gene mutation testing. Conducted at Beijing Haidian Hospital with 126 patients, the study found a 60% malignancy rate for AUS/FLUS nodules and an 87.93% BRAF V600E mutation rate in papillary thyroid carcinoma (PTC) cases. The BRAF V600E mutation demonstrated high specificity and positive predictive value in diagnosing PTC within the AUS/FLUS category, significantly improving detection rates (P = 0.028). The findings suggest that combining preoperative FNA and BRAF V600E testing enhances diagnostic accuracy for indeterminate thyroid nodules, supporting more informed clinical decision-making.
