An immune window of opportunity to prevent spontaneous abortion: prepregnancy peripheral leukocytes and subsets were associated with a decreased risk of spontaneous abortion.

Abstract

This article investigates the association between prepregnancy peripheral leukocytes (PPLs) and their subsets with the risk of spontaneous abortion (SAB) in a large retrospective cohort of over 1 million reproductive-aged women from the National Free Pre-pregnancy Check-up Project (NFPCP) in China. The study found that leukopenia and neutropenia before conception were associated with an increased risk of SAB, while leukocytosis, neutrophilia, and monocytosis were linked to a decreased risk. Additionally, lymphocyte ratio and elevated eosinophils and basophils showed positive associations with SAB risk. These findings highlight the potential importance of immune status prior to conception in SAB pathogenesis and suggest that screening and immune-based interventions before pregnancy could help mitigate SAB risk.
