Hemodynamic Variability During Drainage of Large Volumes of Malignant Ascites in Patients With Cancer.

Abstract
This study investigated whether the variability in mean arterial pressure (MAP) during drainage of malignant ascites differs between large (5–10 L) and small (<5 L) volume removal in cancer patients undergoing cell-free and concentrated ascites reinfusion therapy (KM-CART). Results from 49 patients showed equivalence in MAP variability within ±20% between the two drainage volumes when performed under gravity flow at slow speed, indicating that removal of up to 10 L of ascites does not significantly affect MAP. Other hemodynamic and respiratory variables also showed no significant differences, supporting the safety of large-volume drainage using this method. These findings provide clinical guidance for safely managing malignant ascites drainage and may inform decision-making to improve patient outcomes and quality of life.
