Delayed Pressure Urticaria Associated With Altitude Chamber Training Responsive to Cyclosporine and Omalizumab.

Abstract
This article focuses on a case report of delayed pressure urticaria (DPU), a subtype of chronic inducible urticaria characterized by wheal formation 30 minutes to 24 hours after localized pressure. It describes a military flight crew member who developed DPU following rapid decompression during altitude chamber training, with symptoms refractory to antihistamine monotherapy. Symptom control was achieved through combination therapy including a second-generation antihistamine, leukotriene receptor antagonist, and immunosuppressant cyclosporine, followed by successful management with omalizumab, a monoclonal antibody targeting free immunoglobulin E. The case highlights the challenges in diagnosing and treating DPU, the potential role of altitude changes as a trigger, and the use of advanced therapies to enable return to active duty.
