Adrenal Gland Pathology Reporting Among Genitourinary Pathologists: An Orphan Field Handled by Foster Pathologists?

Abstract
This article examines the current practices and perceptions of adrenal gland pathology among genitourinary (GU) pathologists, who frequently handle adrenal specimens due to their anatomical association with the kidney and surgical removal by urologists. Based on an international survey of 128 GU pathologists, most adrenal specimens are currently managed by GU pathology (56%), though a majority of respondents (74%) believe endocrine pathology specialists should oversee these cases, reflecting concerns about diagnostic complexity and limited formal training. Key challenges include low specimen volume, limited availability of dedicated endocrine pathologists, and evolving diagnostic criteria, which contribute to variable comfort levels in reporting adrenal pathology. The study suggests that expanding endocrine pathology services could improve diagnostic confidence and interdisciplinary communication in adrenal gland specimen evaluation.
