Abstract
This article focuses on evidence-based strategies to prevent lower-limb complications from prolonged use of the dorsal lithotomy position during genital gender-affirming surgery (gGAS). The study from Cedars-Sinai Medical Center describes a specific patient positioning technique using foam padding and regular intraoperative leg checks to minimize compression and injury risk during lengthy surgeries averaging over 8 hours. Among 227 patients undergoing 310 gGAS procedures, only 2.6% experienced transient lower-extremity pain, with no major or chronic complications reported. The authors suggest that their approach effectively reduces postoperative morbidity and recommend its broader adoption in surgical practice.
