Nitrofurantoin: what is the evidence for current UK guidance?

Abstract

This article examines international clinical guidelines on the use of nitrofurantoin for treating uncomplicated lower urinary tract infection (UTI) in non-pregnant women, highlighting significant global variation in recommended treatment durations. UK guidelines from the National Institute for Health and Care Excellence (NICE) and the Scottish Intercollegiate Guidelines Network (SIGN), along with Norwegian and Finnish guidance, uniquely recommend a 3-day course of nitrofurantoin, despite a lack of direct clinical trial evidence supporting this duration at current dosages. Most other international guidelines advocate for longer courses, typically 5 to 7 days, based on limited but more established evidence. The article underscores the uncertainty in extrapolating antibiotic course lengths across different drugs and patient responses, emphasizing the need for further research, such as the ongoing NIHR-funded DurATIon-UTI trial, and suggests that individualized treatment durations may better optimize clinical outcomes and antimicrobial stewardship.
