The resolution of endoperiodontal lesions: New treatment protocol and time sequencing.

Abstract

This article focuses on a new interdisciplinary treatment protocol for endoperiodontal lesions (EPLs), which are infections involving both the dental pulp and periodontal tissues, often resulting in deep periodontal pockets and complex clinical challenges. The protocol reduces the traditional waiting period between nonsurgical periodontal and endodontic treatment and subsequent periodontal regeneration surgery from several months to six weeks. Two clinical cases of advanced EPLs without root damage were treated using this approach, combining one-stage root canal therapy with minimally invasive periodontal regeneration techniques employing enamel matrix derivative, bovine bone mineral, and collagen membranes. After follow-ups ranging from 12 to 42 months, both cases showed significant improvements in periodontal pocket depth reduction, clinical attachment level gain, and radiographic bone fill, suggesting that shortening the interval between treatments may enhance healing and reduce the risk of reinfection.
