Abstract

The article focuses on the role of pharmacists in enhancing palliative and end-of-life care, as outlined in the Royal Pharmaceutical Society’s (RPS) 2025 policy document, *Palliative and End of Life Care: Pharmacy's contribution to improved patient care*. It emphasizes that pharmacists’ expertise in medicines optimization is crucial for effective symptom management, safe medication use, and person-centered care during the last year of life. The document highlights the importance of interprofessional collaboration among pharmacists, nurses, doctors, and other care professionals to prevent suffering, support anticipatory planning, and enable patients to remain in their preferred care settings. Pharmacists also contribute by educating staff, patients, and families about medications, thereby reducing anxiety and improving overall care quality. Integrating pharmacy services into multidisciplinary care pathways is presented as a means to improve symptom relief, care coordination, and patient and family experiences at the end of life.
