Abstract

This article focuses on the implementation and evaluation of a shortened cognitive rehabilitation (CR) programme as a post-diagnostic support for people living with dementia. CR aims to help individuals set and achieve personally meaningful goals to maintain skills and independence, using strategies tailored to cognitive difficulties and lifestyle changes. The programme, structured around the "My Life, My Goals" manual, showed promising improvements in goal attainment and positive participant feedback, though challenges included low referral rates, client insight and motivation issues, and difficulties accessing online materials. The author emphasizes the need for clear referral pathways, multidisciplinary collaboration, carer involvement, and accessible resources to enhance the feasibility and effectiveness of CR in dementia care.
