Abstract
This article focuses on the development and evaluation of a medication use process (MUP) for extemporaneously compounding venetoclax suspensions to administer the oral oncology drug venetoclax to patients with dysphagia or those requiring enteral feeding tubes. Venetoclax, a selective B-cell lymphoma-2 (BCL-2) inhibitor used to treat hematologic malignancies such as acute myeloid leukemia (AML), chronic lymphocytic leukemia (CLL), and acute lymphocytic leukemia (ALL), is typically given as a solid oral formulation but may need reformulation for patients unable to swallow tablets. The study reviews clinical outcomes of 17 patients treated with compounded venetoclax suspensions from 2020 to 2024, highlighting four cases complicated by tumor lysis syndrome (TLS), an oncologic emergency associated with venetoclax therapy. The findings suggest that the standardized MUP provides a practical approach to maintain venetoclax therapy without interruption in dysphagic patients, though further research is needed to clarify pharmacokinetic differences and optimize dosing for enteral administration.
