New horizons in personality disorders—from neglect to necessity in geriatric care.

Abstract
This article focuses on the recognition, diagnosis, and management of personality disorders in older adults, emphasizing their prevalence, impact on geriatric care, and the need for age-inclusive approaches. Personality disorders, characterized by enduring maladaptive patterns of behavior and cognition, affect about 10% of older adults and are linked to increased somatic multimorbidity and healthcare utilization, yet often remain underdiagnosed and inadequately treated in geriatric settings. The article highlights challenges in diagnosis due to age-related changes and symptom overlap, advocates for age-neutral diagnostic tools, and reviews evidence for psychotherapeutic and pharmacological treatments tailored to older populations. It calls for integrated, interdisciplinary care models and innovative research—including real-time monitoring technologies and inclusion of diverse cultural perspectives—to improve outcomes and quality of life for older adults with personality disorders.
