This article addresses the complexities of assessing Attention Deficit Hyperactivity Disorder (ADHD) in children who have experienced developmental trauma, highlighting the significant overlap and co-occurrence between these two presentations. It outlines the diagnostic criteria and assessment methods for ADHD, as defined by the DSM-5 and ICD-11, and contrasts these with the effects of early adversity and developmental trauma, which impact brain development, emotional regulation, and behavior but are not currently formal diagnostic categories. The authors emphasize the challenges clinicians face in differentiating ADHD symptoms from trauma-related behaviors, advocating for comprehensive, trauma-informed assessments that incorporate detailed developmental histories, multi-source information, and careful clinical formulation rather than relying solely on diagnostic checklists. The paper concludes with practical guidance for clinicians to consider environmental context, timing, and the child's unique experiences to avoid misdiagnosis and ensure appropriate support.
