Abstract
This article focuses on a case study of a 36-year-old male with bilateral testicular masses, refractory testicular pain, non-obstructive azoospermia, and Klinefelter syndrome who underwent bilateral oncologic testicular sperm extraction (Onco-TESE) and placement of bilateral testicular prostheses. Preoperative evaluations included physical examination, scrotal ultrasound, hormonal and genetic testing, and semen analysis. Pathology revealed multinodular Leydig cell hyperplasia, and no viable sperm were retrieved; however, the patient’s pain resolved postoperatively with satisfactory cosmetic outcomes. The case demonstrates the feasibility of bilateral Onco-TESE combined with prosthesis placement as a therapeutic approach aimed at preserving future fertility in complex clinical scenarios.
