Abstract
The article focuses on the Southern Arizona Hypertension Control Collaborative (SAHCC), a 2023 partnership between six federally qualified health centers (FQHCs) and three local health departments (LHDs) in Southern Arizona aimed at addressing unrecognized and undertreated high blood pressure. SAHCC integrates clinical best practices, community awareness campaigns, enhanced patient linkage to primary care, and coordinated public health strategies to improve hypertension control across Cochise, Pima, and Santa Cruz counties. Key activities include sharing treatment protocols among FQHCs, public education efforts, establishing hotlines for patient navigation, and using geo-mapping to target high-risk areas. Early outcomes show improved blood pressure control rates in participating health centers and increased community engagement through screenings and educational programs. The collaborative model emphasizes sustainable partnerships leveraging the complementary strengths of FQHCs and LHDs to enhance regional cardiovascular health.
