The article focuses on the impact of implementing a counselling checklist for patients prescribed sodium-glucose co-transporter 2 (SGLT-2) inhibitors across four hospital wards. Over six months, 110 patients were newly prescribed SGLT-2 inhibitors, with increased patient education observed post-implementation, including a rise in distribution of medicine information books and 'protect my kidney' cards from 25% to 70%, and counselling rates from 27% to 70%. Documentation of prescribing indications and specialties improved, with unknown cases decreasing from 3% to 0%. The study suggests that using counselling checklists enhances patient communication and the quality of transfer of care records, highlighting their importance in discharge processes despite the study's limited duration and sample size.
