Post-implementation optimization of medication alerts in hospital computerized provider order entry systems: a scoping review.

Abstract

This article focuses on interventions for optimizing hospital medication alerts after their implementation, based on a scoping review of 16 studies primarily from the United States. The review identified six main types of optimization strategies—alert inactivation, alert severity reclassification, information provision, use of contextual information, threshold adjustment, and encounter suppression—that generally reduced alert quantity and improved clinical acceptance and utility. Most studies involved inpatient settings and targeted alerts such as drug–drug interactions, drug dosage, and drug allergy alerts, with outcomes showing decreased alert volume and increased alert acceptance, though impacts on clinical care and alert fatigue were less frequently measured. The review highlights the heterogeneity of study designs and interventions, the predominance of observational methods, and the need for standardized endpoints including patient safety and clinician outcomes in future research.
