Effect of Antiviral Treatment on Hepatitis B Virus Integration and Hepatocyte Clonal Expansion.

Abstract

This article focuses on the impact of nucleos(t)ide analogue (NUC) treatment on hepatitis B virus (HBV) DNA integration and hepatocyte clonal expansion in patients with chronic hepatitis B (CHB), both of which are implicated in hepatocellular carcinoma (HCC) development. In a study of 28 patients treated with NUCs (lamivudine, telbivudine, or entecavir), HBV DNA integration and hepatocyte clone size were detectable at baseline and progressively decreased over 1 to 10 years of treatment, despite persistent integration. The reduction in HBV integration and clonal expansion occurred independently of changes in standard virologic markers, suggesting that NUCs may lower HCC risk through mechanisms beyond direct viral suppression. These findings support long-term NUC therapy as a strategy to reduce HCC risk by interfering with HBV DNA integration and hepatocyte clonal expansion.
