Association between urinary C4d levels and disease progression in IgA nephropathy.

Abstract
This article focuses on the evaluation of urinary C4d as a non-invasive biomarker for predicting renal outcomes and treatment response in immunoglobulin A nephropathy (IgAN). In a cohort of 508 biopsy-proven IgAN patients, higher urinary C4d levels were independently associated with worse kidney function, more severe proteinuria, and increased risk of kidney progression events, defined as a 50% decline in estimated glomerular filtration rate, end-stage kidney disease, or death. Additionally, elevated baseline urinary C4d predicted a poorer proteinuria response to steroids or immunosuppressive therapy. The findings suggest that urinary C4d reflects lectin pathway activation in IgAN and may serve as a useful tool for risk stratification and monitoring therapeutic response, although further validation in diverse populations is needed.
