Abstract
This article focuses on the retrospective radiologic and clinical evaluation of full-arch immediate fixed prostheses supported by custom-made, 3D-printed subperiosteal titanium implants in patients with severely atrophic jaws. Using the Stability, Infection, and Offset (SIO) criteria, the study assessed implant success, survival, and failure in 18 patients over a two-year follow-up, analyzing implant positioning accuracy via CT and CAD/CAM digital planning. Results showed a 73.1% implant success rate and 100% prosthesis survival, with transient implant micromotions and soft tissue recession classified as survival rather than failure. The authors propose the SIO framework as a standardized method to objectively evaluate subperiosteal implant outcomes, emphasizing the importance of precise virtual planning and spatial offset thresholds to optimize clinical success in cases unsuitable for conventional endosseous implants.
