The effects of remifentanil and dexmedetomidine on the gastro-oesophageal pressure gradient: A crossover randomised controlled volunteer trial.

Abstract

This study compares the effects of remifentanil, a pure opioid agonist, and dexmedetomidine, a selective alpha-2 adrenergic receptor agonist, on the lower oesophageal sphincter pressure (LOSP) and gastro-oesophageal pressure gradient (GOPG) in healthy volunteers. Using a randomized, double-blinded crossover design, both agents were found to cause similar, dose-dependent decreases in LOSP and GOPG, with reductions up to approximately 65% at higher sedation levels. These decreases suggest an increased risk of gastro-oesophageal reflux and potential aspiration during sedation with either drug. The findings highlight that the choice between remifentanil and dexmedetomidine for sedation should not be based on their effects on oesophageal pressure or reflux risk, and caution is advised when using these agents in patients with unsecured airways.
