Petrous Carotid Artery Thrombosis in an Immunocompromised Patient Presenting With Mastoiditis, A Case Report.

Abstract

This article focuses on a rare case of internal carotid artery (ICA) thrombosis causing a middle cerebral artery (MCA) infarct in a patient with acute mastoiditis. The patient, a renal transplant recipient on immunosuppressive therapy, presented with subtle otologic symptoms and developed a stroke due to thrombosis adjacent to a dehiscent petrous carotid artery exposed by mastoid infection. The case highlights that immunocompromised patients may lack typical signs of mastoiditis, necessitating prompt surgical intervention alongside antibiotics and anticoagulation to resolve thrombosis and improve neurological outcomes. The authors emphasize aggressive management of mastoiditis in immunosuppressed individuals to prevent both venous and arterial thrombotic complications.
